To receive a SIM card, we will require a referral form from a third party such as a charity, support organisation, community group, school, GP surgery, social worker, housing association, advice centre, or any professional agency that can confirm your circumstances.

Must be 18+
Has limited or no access to the internet at home or when away from home
Cannot afford internet costs (low-income household or unable to maintain a contract/top-up)
